Medication at School

  • All medication to be given at school, including prescription and over-the-counter medication, require a written permission from the prescribing physician and the parent/guardian. (Use medication consent form PH-17a)
  • All medication must be delivered to the school nurse in its original container.
  • Students are not permitted to carry and deliver medication to school.
