Dear Parents and Guardians,

We acknowledge the challenges faced this past school year in sending required health documentation to school. Please see below for the required forms and documentation for your student(s). These documents which are required by the state are one of the ways we ensure your child is safe here at school.

Kindergarten, 1st Grade, and New Students: Physical form, dental form, and updated immunizations
2nd Grade: All forms not yet submitted from PAELC- physical form, dental form, and updated immunizations
3rd Grade: Dental form
4th Grade: Any 3rd grade dental form not submitted the previous year 
5th Grade: Any forms or immunizations due from previous years
6th Grade: Required physical form and updated immunization records
7th Grade: Record of required Tdap and MCV vaccines and dental form
8th Grade: Any forms not yet submitted from last year and updated immunization records
9th Grade: Any forms not yet submitted from Middle School, especially record of MCV and 
Tdap vaccines typically given in 7th grade
10th Grade: Any forms not yet submitted from last year and updated immunization records
11th Grade: Required physical form and updated immunization records
12th Grade: Record of required second MCV vaccine

o Physical and dental exams performed by your physician within 12 months prior to the first day of school are acceptable as are sports physicals. If you prefer to have your child examined by the school physician and/or school dentist, the exam will be conducted sometime during the upcoming school year.

o If your child has received any immunizations within the past year, please submit those as well for our records. Please reference this page for the PA Department of Health vaccination requirements.

Additionally, if your child receives vaccination for COVID, please send proof of this to the School Nurses’ office of your student’s school building. Accurate identification of vaccinated students is especially relevant if contact tracing is required to be conducted in school. 

o All physical forms, action plans, and medication forms can be found on each school’s website under the Health Services Department tab. Please submit required documentation within the first week of school to the Nurses’ Office if these apply to your child.
 
o Please also ensure that your child’s 2021-2022 Health Form is completed in Skyward. This provides us with your permission to administer treatment your child during the school day

With the beginning of a new school, we would like to remind you of a few tips to help facilitate your child's academic success:

1. Eat healthy/Eat breakfast/Eat brain food:
    ~choose fruits, veggies, nuts and seeds
    Avoid fatty and sugary snacks
    Focus on getting a variety of vitamins in their diet

2. Drink more water

3. Exercise Everyday:
    An active body is part of a healthy mind
    Turn off the TV and Computer and get active
    
4. Get plenty of Rest:
    Get 8-10 hours of sleep a night

Important Information
 
(1) Mandated Screenings Performed At School
 
During the school year, we will assess your child's height and weight, calculate their BMI, and screen their vision. In addition, a hearing screening will be given to all 7th graders.  A referral will be sent home if further evaluation is recommended for these screenings.
 
A scoliosis screening (an examination of the spine) will be given to 7th graders, a referral will be sent home if further evaluation is recommended.
 
(2) Physical and Dental Exam Requirements
 
Physicals
According to Pennsylvania School Health laws, Students entering 6th grade or new to our school district he/she need to have a physical exam completed.  The physical exam time is a perfect opportunity for discussion with your child's doctor about any additional immunizations they may need.  The exam should be completed and returned to our office by Thanksgiving break.

Reminder: Students entering 7th grade are required to have meningitis (MCV), Tdap booster and two doses of the chickenpox vaccine prior to the first day of school.  Please forward their updated immunization record to your school nurse.
 
Dental
According to Pennsylvania School Health Law, Students entering 7th grade or new to our school district he/she need to have a dental exam completed.  Please have the completed dental form to our office by Thanksgiving break.
